Before you come in — a few quick notes

Patients under 18
Must be accompanied by a parent or legal guardian who stays for the entire visit and signs consent. Please bring a photo ID for the guardian.

Pregnant patients
Please bring a written clearance from your OB-GYN. Without it we cannot take X-rays, give anesthetic, or perform the extraction.

Blood thinners or bone density meds
A $49 limited evaluation + x-ray visit is required first so we can review your medical history and coordinate with your physician. Skipping this step usually delays care.
